thinkcatalyst was founded by a team of physicians who created a suite of analytic tools to help our primary care and specialist clinical colleagues intensely monitor disease conditions and detect data trends that may allow for earlier interventions in therapy.  By keeping patients healthier longer, and intervening clinically at an earlier stage in a disease process, we allow patients to prevent exacerbations of symptoms that may necessitate higher levels of care (ie. emergency or ICU care).   This model of team based virtual care using robust real-time analytics and patient engagement provides an alternative to the costly traditional "meet and treat" model.  The process can begin the moment a patient leaves the hospital or from a referral from the community.  Below is an example of how the thinkcatalyst workflow could work for a patient discharged from the hospital, without having the benefit of a family doctor in the community.
